Shore Puppy Walks Service Agreement Contract - Dog Walking/Pet Sitting
The parties herein agree as follows:
This contract will take effect upon signature by both Client and Pet Sitter and will remain in effect until terminated by either party as provided below in Item 9.
Pet Sitter is authorized to perform care and services as outlined on this contract. Both Shore Puppy Walks and Client recognize that the welfare of the animal is the highest priority. If in Shore Puppy Walk's judgment additional services become necessary during the service period to properly care for the animal, Shore Puppy Walks will first make reasonable attempts to contact Client. If Client cannot be contacted for whatever reason, Shore Puppy Walks is authorized to undertake such additional steps as may in the reasonable judgment of Shore Puppy Walks be necessary or appropriate for the health and welfare of the animal, including but not limited to (a) additional visits by Shore Puppy Walks to provide care for the animal; (b) consultation with Client's Veterinarian listed above, or with an emergency veterinary care provider should Client's Veterinarian be unavailable; (c) authorizing care and treatment as recommended by Client's Veterinarian or an emergency veterinary care provider (excluding euthanasia) and (d) such other steps as may in the reasonable judgment of Shore Puppy Walks be necessary or appropriate for the health and welfare of the animal. Client agrees to be responsible for all fees and expenses incurred for care and treatment of the animal pursuant to this paragraph, and releases and holds Pet Sitter harmless from all liabilities related to transportation, treatment and expense. Client agrees to reimburse Shore Puppy Walks for any expense incurred, plus any additional fees for attending to animal's needs or any expenses incurred for any other home/food/supplies needed.
In the event of inclement weather, natural disaster or circumstances beyond our control, e.g. acts of terrorism, etc., Shore Puppy Walks is entrusted to use best judgment in caring for pet(s) and home. Shore Puppy Walks will be held harmless for consequences related to such decisions.
Shore Puppy Walks agrees to provide the services stated in this contract in a reliable, caring and trustworthy manner. In consideration of these services and as an express condition thereof, CLIENT EXPRESSLY WAIVES AND RELINQUISHES ANY AND ALL CLAIMS AGAINST SHORE PUPPY WALKS ARISING OUT OF OR RELATING TO THE PROVISION OF SERVICES HEREUNDER, EXCEPT THOSE ARISING FROM GROSS NEGLIGENCE OR WILLFUL MISCONDUCT ON THE PART OF PET SITTER/COMPANY. SHOULD SHORE PUPPY WALKS OR ANY AUTHORIZED PERSON ACCOMPANYING SHORE PUPPY WALKS SUSTAIN ANY INJURY, DISEASE OR OTHER HARM IN THE COURSE OF PROVIDING SERVICES HEREUNDER, CLIENT WILL INDEMNIFY SHORE PUPPY WALKS AND HOLD IT HARMLESS WITH RESPECT TO ALL LOSS, EXPENSE AND DAMAGE CAUSED THEREBY, EXCEPT THOSE ARISING FROM GROSS NEGLIGENCE OR WILLFUL MISCONDUCT ON THE PART OF SHORE PUPPY WALKS.
Client acknowledges that payment is due in full on the date of the first visit. In the event it is necessary to initiate collection proceedings on the account, Client will be responsible for all attorney's fees and costs of collection.
In the event of personal emergency or illness of Shore Puppy Walks, Client authorizes Shore Puppy Walks to arrange for another qualified person to fulfill responsibilities as set forth on this contract. In such case, Shore Puppy Walks will remain fully responsible for the proper discharge of all services under this Agreement. Every attempt will be made to notify client regarding such situation.
All pets are to be currently vaccinated.
Shore Puppy Walks and Client each may terminate this contract at any time by written notice to the other. Shore Puppy Walks will be entitled to payment for all services rendered until notice of termination is received, and for any transition services reasonably required to provide for the health and welfare of Client's pets. Shore Puppy Walks will not terminate during a period of scheduled service unless Shore Puppy Walks determines, in his/her sole discretion, that a danger exists to the health or safety of Shore Puppy Walks. If such concerns preclude Shore Puppy Walks from providing further care of the pet, then Client authorizes pet to be placed in a kennel, with all charges therefrom to be charged to Client. Every attempt will be made to notify Client regarding such situation.
Client acknowledges that by signing below, he/she is providing written approval for the provision of services by Shore Puppy Walks during any service period scheduled by Client and accepted by Shore Puppy Walks. Upon such scheduling and acceptance, Shore Puppy Walks will be authorized to enter Client premises and perform services without additional signed contracts or written authorization and to accept telephone reservations for future visits.
Please note: Due to a pet's excitement to see us, “door darting” may be attempted by a pet. We take every precaution to prevent this from occurring, but do require that all dogs and cats under our care wear an ID tag (or collar) stating their name and your phone number.
I have reviewed this Service Contract in its entirety. The information provided by me is complete and accurate and I agree to all its terms and conditions as set out above.

Shore Puppy Walks Service Agreement Contract - Doggie Daycare

This Shore Puppy Walks, LLC (SPW) Day Camp & Boarding Agreement is made on the date of signature below between Shore Puppy Walks, LLC (hereinafter “SPW”) and the pet owner (hereinafter “Owner”), and hereby agrees to be bound by the following:

Whereas, SPW is a dog day care and boarding facility located at 18 Furlong Drive, Cape May Court House, NJ 08210.

Whereas, Owner is the lawful pet owner of dog(s) named and scheduled under registration account using Precise Pet Care.

NOW, THEREFORE, in consideration of good and valuable consideration, the receipt of which is hereby acknowledged, and said promises and the mutual covenants and agreements set forth in this Agreement, the Parties hereto, intending to be legally bound, hereby agree as follows:
Shore Puppy Walks, LLC Facilities. SPW is a dog daycare and boarding facility specializing in the care of dogs. SPW provides an environment for dogs to socialize, play, romp, and run in supervised and closed off quarters. SPW provides both indoor and outdoor facilities. Indoor and Outdoor facilities are provided to ensure the dogs are kept in closed off and safe areas to play in the event of severe heat, rain, thunderstorm, or other unexpected weather events.
Hours of Operation.
Boarding Hours of Operation. The facilities are open from 7:00 am to 5:00 pm Monday - Friday and 8:00 am - 5:00 pm on Saturdays. The Facilities are open from 8:00 am to 10:00 am on Sundays for pick up only.
Daycare Hours of Operation. The Facilities are open from 7:00 am to 5:00 pm Monday - Friday and 8:00 am - 5:00 pm on Saturdays. Daycare IS NOT open on Sundays.
Scope of Services and Warranties.
Boarding Scope of Services. Included in boarding is time for the Owner's dog(s) to play at daycare for the duration of one day. Owner will provide instructions for feeding, along with food. Owner will provide instructions for sleeping, along with crate/bedding. SPW will make every attempt to allow dog(s) to use the regular sleeping habits of the dog(s) as instructed by the owner. SPW reserves the right to provide a crate to sleep in if deemed necessary for the safety of the dog(s).
Daycare Scope of Services. Included in daycare is unlimited access to outside and indoor facilities. Dog(s), at the sole discretion of SPW, will have the freedom to move between the two areas during the day.
SPW Warranties. SPW does not warrant to Owner specialized medical knowledge and/or treatment of dogs. SPW does not maintain veterinarians, animal surgeons, or licensed practitioners that can provide needed treatment or operate on dogs on the premises. SPW does not warrant to Owner to have any animal behavior specialist or any advanced expert knowledge on dogs.
Payment by Owner
Boarding Payments. Owner understands and agrees that prices for boarding are due in full on the first day of the visit and include the entire duration. Boarding reservations that fall over named holidays incur an additional fee.
Daycare Payments. Owner understands that payment is due in full on the first day of visit for the current week, prior to use of the facility.
Items Owner May Leave with SPW for Boarding/Daycare.
Owner Supplied Food. SPW understands that certain dogs may have special dietary needs. It is the Owner's sole responsibility to notify SPW of these specialized needs and any allergies to other products, food, or treats.
Comfort Items. Owner may want to supply comfort items while boarding at SPW. Comfort items may include: toy, bed, crate, treats, blankets, bowls. Owner understands and agrees that SPW is not responsible for the wear or destruction of these comfort items used by the Owner's dog(s) while boarding at SPW.
Owner's Promises & Covenants to SPW.
Owner's Promises and Covenants. Owner acknowledges that SPW, to the best of its ability, practices safety for the health of all dogs and staff. Owner understands that SPW strongly relies on the Owner's promises to SPW regarding Owner's dog(s) health and behaviors. Owner understands that safety is top priority at SPW and during all times of this Agreement, Owner agrees to be candid and forthcoming regarding Owner's dog(s) behavior and health. Owner understands and agrees that SPW WILL NOT accept dogs with the following: tick/fleas, female dogs in heat or out of heat within one month, known aggressive tendencies towards other dogs or humans, history of one or more dog bites, a lapse in medical records, expired vaccinations, dogs less than four months of age.
Aggressive Dog Behavior. If, at any time, SPW, in its sole discretion, determines thatOwner's dog(s) is/are aggressive toward SPW staff, other dogs, and/or other owners, SPW has the absolute right to demand Owner retrieve Owner's dog(s) immediately. Aggressive behavior is NEVER tolerated at SPW and Owner hereby agrees to be available to pick-up his/her dog immediately. If Owner cannot be reached for whatever reason,
SPW has the right to contact emergency references OR transport Owner's dog(s) to the veterinarian disclosed on Owner's records. Owner understands and agrees that SPW is not responsible for any cost incurred at the veterinarian.
Females in Heat. If Owner's dog(s) go into heat while at SPW, Owner agrees to be available to pick up immediately. Owner also understands that if Owner's dog(s) is/are not spayed, that Owner places such dogs at SPW at Owner's own risk. If at any time Owner's dog(s) goes into heat and becomes pregnant while at SPW, Owner agrees to indemnify and hold harmless SPW from any suit, claim, demand, or charge for medical bills as a result of the pregnancy.
Owner's Duty to Notify Love My Dog & Keep Updated Records.
Veterinary Records. Owner must provide an updated shot record printout from Owner's veterinarian prior to boarding. The records must indicate current Rabies, DHPP, and Bordetella vaccinations. Bordetella vaccination MUST be administered 14 days prior to daycare and/or boarding. Owner hereby agrees to provide any and all updates to Owner's dog(s) veterinary records. It is the Owner's sole duty to provide SPW with all updates. SPW is not responsible for any accident, injury, or medical condition as a result of pre-existing conditions to Owner's dog(s).
VIII. SPW Right to Seek Veterinarian Help on Behalf of Owner's Dog(s) Without Owner's Consent.
Owner agrees and understands that SPW shall, without Oner's consent, have the right to seek emergency veterinarian help on behalf of Owner's dog(s) in the event of an emergency. Owner consents to have SPW transport the Owner's dog(s) to an animal hospital for immediate treatment in the event SPW determines Owner's dog(s) need immediate attention. The determination of immediate medical attention is hereby stipulated to as reasonable and Owner agrees to be responsible for any additional costs associated with such emergency veterinarian or animal hospital help.
X. Daycare/Boarding Conditions to Utilizing Facilities: Pick-up and Drop-Off.
Drop Off. Owner's shall drop off dog(s) between requested time and 10 am. After 10 am, Owner must contact SPW to schedule a drop off time. Owner agrees to secure dog(s) with an appropriate harness or martingale collar. Owner agrees to hand-off Owner's dog(s) to a SPW staff member. Owner understands and agrees that Owner is NOT permitted to enter into the main building for the safety of the Owner and the safety of the other dogs.
Pick Up. Owner agrees to promptly pick up dog at the scheduled time. If the Owner picks up later than scheduled time, Owner will be charge $4.00 for each additional hour until closing. Any dog(s) still at daycare/boarding after closing (5:00 pm) will result in a full day's charge. Owner understands and agrees that Owner is not permitted to retrieve Owner's dog(s) on their own and must have a SPW staff member hand off Owner's dog(s) to Owner at the time of pick-up. Once Owner has Owner's dog, SPW is no longer responsible for Owner's dog(s).
XI. Owner Agrees to Be Responsible for Act's of Owner's Dog(s).
Owner's Responsibility. Owner agrees to be solely responsible for any and all acts of Owner's dog(s) while in the care of SPW. Owner agrees to pay for and be solely responsible for any injury or death to SPW staff, other dogs, and/or damage to SPW facility. Owner is hereby responsible for any losses or damage suffered by SPW as a result of Owner's failure to inform SPW of any pre-existing condition or behavior (such as illness or aggression) which were otherwise caused by Owner's dog(s).
XII. Owner Release & Indemnification Of SPW.
Release of Transferrable Conditions & Illnesses. Owner acknowledges that SPW provides day care for many dogs. Owner understands and agrees that although SPW practices preventative steps from having transferable bacterial and viral conditions from spreading, such complete prevention is not possible. Owner understands and agrees that although SPW obligates owners to practice safety and inform SPW of changes with their dogs, all owners may not abide fully. Owner acknowledges that owners may bring dogs with colds, kennel cough, and bacterial or skin infections that are transferable from dog to dog, without informing SPW. Owner hereby understands and agrees that Owner places Owner's dog(s) at SPW at Owner's risk and that SPW is not responsible for any of the following while at the care of SPW: kennel cough, bordetella, stomach flu, standard flu, bloat, bacterial infections, viral infections, skin infections, fleas, ticks, and any other infections, conditions, parasites, and/or ailments that are transferable from dog to dog, whether internal or external.
Release of Injury or Death. Owner agrees that SPW will not be responsible for death or damage caused by illness or disease, natural disasters, extreme escape measures, aggressive behaviors or normal play that results in injury or death, or malfunction of restraint gear that results in injury or death. Owner hereby agrees not to bring any suit, claim, charge, demand, rights, and causes of action whatsoever against SPW and its respective officers, directors, employees, and agents arising from, arising out of, or in connection with the use of SPW facilities, in any way whatsoever now and in the future.
Indemnification. Owner will indemnify, save harmless, and defend SPW promptly and diligently at Owner's sole expense from and against any and all claims and demands in connection with any claim, demand, injury, accident, or damage whatsoever caused to any of Owner's dog(s) or property arising directly or indirectly out of Owner's dog(s) stay at SPW occurring in, on or about the Premises or any part thereof, including the access way and front yard, or arising directly or indirectly from any act or omission of Owner or any of its dogs from and against any and all costs, expenses and liability incurred in connection with any such claim or proceeding brought thereon.
XIII. Entire Agreement. This agreement constitutes the entire understanding and agreement between SPW and Owner, with regard to all matters herein. There are no other agreements, conditions, or representation, oral or written, expressed or implied with regard thereto. This agreement may be amended only in writing and signed by both parties.

IN WITNESS WHEREOF, SPW and Owner have executed this Agreement consisting of six (6) pages as of the date below written, each acknowledging receipt of an executed copy hereof.
